PAYMENT: THERAPY
Check, credit/debit cards, and HSA cards are accepted. Sessions have a fee of $180. Payment is due at the time of service. A sliding scale rate based on income can be discussed when necessary.
Dr. Guides is not in-network with any insurance plans. Some plans reimburse policyholders for part of the cost of a session with an out-of-network provider. If clients have and wish to use these benefits, Dr. Guides provides an insurance-compatible billing statement for them to submit to their insurance company.
PAYMENT: EVALUATIONS
Fees for Evaluations depend on the nature and comprehensiveness of the assessment. Please contact Dr. Guides for more information.
CANCELLATION
If the need to cancel arises, Dr. Guides kindly requests at least 24 hours notice. Unless there are extenuating circumstances, an appointment cancelled with less than 24 hours notice is billed at the full rate.
